Requirements for Pag-IBIG Loyalty Card

Before you apply, make sure to prepare the necessary requirements such as 2 valid IDs, your Permanent Membership ID number(MID), and an active Pag-IBIG membership At least 1 mo. contribution within the last 6 mos.

Applying for Pag-IBIG Loyalty Card

To apply for a Pag-IBIG Loyalty Card, you need to download and fill out the application form, submit it to the nearest Pag-IBIG Fund branch, pay the registration fee of ₱125, and have your biometrics taken.
Make sure to validate all the information you provided and check for errors and misspellings. Once your application is approved, you can claim your Pag-IBIG Loyalty Card Plus and PIN Mailer from the bank personnel.

What is PIN mailer? A PIN mailer for the Pag-IBIG Loyalty Card Plus is a secure method of delivering the Personal Identification Number (PIN) associated with your Pag-IBIG Loyalty Card Plus. The PIN is a unique and confidential code required to access and authorize transactions using your card, such as ATM withdrawals or point-of-sale transactions.

To register for a Pag-IBIG Loyalty Card Plus, follow these simple steps at any Pag-IBIG Fund branch:

1. Complete the Pag-IBIG Loyalty Card Plus Application Form (HQP-PFF-108) (2), which can be downloaded
2. Submit the application form at your nearest Pag-IBIG Fund branch, along with a photocopy of a valid ID card. Present your ID card for verification.
3. Pay the card fee to the designated bank-operator.
4. Have your photo and biometrics captured.
5. Verify your information as recorded by the bank-operator.
6. Once done, you’ll receive your Pag-IBIG Loyalty Card Plus.
